Program Goals and Objectives
Community Involvement
The residents of “Grace House” are supervised and held
accountable to the Parole release conditions and “Grace House” reporting rules.
The residents are subject to the “Grace House” accountability issues defined by
their individual Case Manager. “The Grace House” program is a leader in its
field having created a unique partnership and linkage agreement with the New York State Department of Corrections and Community Supervision. The success of “Grace House” is the unique
partnerships with the religious community, businesses, addiction treatment
program, the Erie County Department of Social Services, various job training
programs (VESID), literacy Volunteers and local employers working together to
provide opportunity and a clear direction of accountability for program
participants to become productive members of society with the life skills
necessary to live independently.
“Grace House” Transitional Residence is licensed by the City
of Buffalo as a group home. The partnership with New York State Parole
provides a State Parole officer 24 hour facility access with allocated office
space. Program participants understand this linkage agreement by the physical
parole presence and enforcement of “Rules” by “Grace House” staff.
Through the outreach of “Grace House” staff, the agency
receives nearly all food products from the regional food bank, a local bakery
and grocery purveyors that regularly donate to the facility. Medical care,
addiction treatment, Parole mandated counseling is provided through the linkage
with the Erie County Medical Center, Mid Erie Mental Health and Horizon Human
Services Mental Health/Addiction Treatment Centers.
Through the experienced agency staff, daily contact with Field
Parole and the numerous community agency collaborations the new target
population will have all food, clothing, and medical needs met as well as
ability to receive spiritual and mental health counseling.
The program affords this client group opportunity to receive
care, both physical and medical, without personal cost. During program
commitment, six months to one year, the primary goals, are participant
stabilization and community safety. Through case management and involvement
with collaborative community partners each participant is screened, assessed and
a transitional program is designed to meet the individual level of care
required. Staff encourages clients to become self supportive, independent and
connected with care providers that assist the individual to live independent and
crime free.
