Case Management Staff
Charise Cobbs
Discharge Planning Coordinator and Case Manager Supervisor
Charise Cobbs joined the SGM staff in October of 2009 as a
Discharge Planning Coordinator. In this role Charise is responsible for the
case management, service coordination, and program oversight of a primary
program caseload. Prior to joining the SGM staff Charise was a Habilitation
Specialist and a Wraparound Mentor for Baker Victory Services, providing
supervision and case management for high-risk children. With her experience
in case management and working with low-income, high risk families and
individuals, Charise is a valuable addition to the SGM case management team.
Charise received her Bachelors degree in communications from Buffalo State
College. In early 2010, Charise became the Case Manager Supervisor,
overseeing all case management staff and activities. |

Vincent Phillips
Aftercare Housing Case Manager
Vincent Phillips joined the SGM, Inc. staff in July of
2008 as a case manager. Vincent currently serves as the agency’s Aftercare
Housing Case Manager, assisting graduating clients in locating housing,
employment, and accessing other services, and providing aftercare follow-up
and assistance for up to one year after completion of the primary “Grace
House” program. Vincent is a valuable addition to the case management team,
able to locate housing for even the hardest-to-place cases. |

Lisette Gutierrez
HPRP Program Case Manager
Lisette Gutierrez joined the staff in August of 2008 as
the Office Administrator. In that capacity, she was responsible for
overseeing all client applications for public benefits and entitlements and
tracking client progress through program. In January of 2010, Lisette became
the agency’s Homelessness Prevention and Rapid Re-Housing (HPRP) Case
Manager. She is responsible for completing and processing HPRP client
intakes and assessments, and performing the initial evaluation for
eligibility for HPRP services under the federal HUD stimulus contract. |

Cielito Garcia
Youth Advocate Case ManagerCielito
Garcia joined the staff in August of this year as the Youth Advocate Case
Manager. In that capacity, she will be responsible for overseeing case
management for clients age 25 and under, as well as covering a portion of
the female client caseload. Cielito has several years of case management
experience, most recently at Spectrum Human Services where she served as a
Reentry Care Coordinator. She brings to the staff a wealth of experience and
knowledge in servicing the hard-to-place, high-risk, high-needs cases. |
