Inmate Intake Selection Process
First: Candidates for resident consideration:
Inmate is referred by New York State Prison Officials; Department of
Corrections or Division of Parole. A Parolee may be referred by the Division
of Parole or by Health Service Agencies. We will not accept referrals from
family members; religious leaders, government officials unless directly
representing the New York State Department of Criminal Justice Services.
Second: Upon receipt of the referral, we will then
forward to the inmate the Intake Assessment questionnaire packet and
required release forms. Note: required forms may be downloaded from web
site. The inmate is expected to complete the required questionnaire
forthrightly and return the completed package to our address.
Third: Upon receipt of the completed Intake
Assessment questionnaire and executed release documents, the Intake
Assessment Team will begin the assessment process. The assessment process is
an internal review of the potential resident candidate documents and the
ordering and review of this criminal history file.
Fourth: If in the opinion of the Intake Assessment
Team the candidate is to be further considered for program, a representative
(agency staff or New York State Department of Corrections and Community Supervision Representative) may elect
to conduct facility interview with potential resident candidate.
Fifth: Assuming candidate is approved to be
accepted to “Grace House”, the Intake Assessment Team notifies the prison
Parole Division and the local Field Parole that the inmate candidate will be
accepted to the program. Program contracts and related agreements are to be
executed by inmate. Copies will be provided to the Division of Parole.
Sixth: Intake Assessment Team awaits notification
by New York State Department of Corrections and Community Supervision approving resident address and inmate
approval to “Grace House” Program. New York State Department of Corrections and Community Supervision
notifies “Agency” of approximate release date of inmate.
